What is isotonic saline 0.9% and what is it used for?

Isotonic saline solution 0.9% is an electrolyte solution that is used for fluid and electrolyte replacement. It contains sodium (Na+) and chloride (Cl-) in a concentration of 0.154 mmol/ml and 154 mmol/l respectively. The solution is used for hypochloremic alkalosis, chloride losses, short-term intravascular volume replacement, hypertonic dehydration and isotonic dehydration.

What should you consider before using isotonic saline solution 0.9?

An absolute contraindication exists in states of overhydration (hyperhydration).Relative contraindications include decreased blood potassium (hypokalemia), increased blood sodium (hypernatremia), increased blood chloride (hyperchloremia), and conditions requiring restrictive sodium intake, such as heart failure, generalized edema, pulmonary edema, hypertension, eclampsia, and severe renal insufficiency.Electrolyte and fluid status checks are required. To date, there are no known interactions with other medications. There are no concerns about use during pregnancy and breastfeeding.

How to use isotonic saline solution 0.9?

The dosage depends on the fluid and electrolyte requirements, which are usually 40 ml/kg body weight/day or 2 mmol sodium/kg body weight/day.The maximum infusion rate and daily dose are determined by the clinical situation and fluid and electrolyte requirements. In cases of hypertonic dehydration, excessive infusion rates should be avoided. The solution is infused intravenously. In the event of an overdose, overhydration, increased sodium and chloride content of the blood, hyperosmolarity and an acidotic metabolic state can occur.Therapy includes interruption of the supply of the solution, accelerated elimination via the kidneys and a reduced supply of the corresponding electrolytes.
