The Arabin cube pessary is a medical aid made of tissue-friendly silicone.It has a button with a firmly anchored retaining thread that makes changing easier.The pessary can be stored at room temperature between 1 and 30 °C, but should be protected from UV-radiation and direct contact with reactive media such as gas, ozone or mineral oil.

The cube pessary is perforated and available in size 2 with a diameter of 32 mm.

Scope of application

The Arabin cube pessary is used to treat various degrees of vaginal prolapse.In the event of a prolapse in which the vagina is partially turned out, the pessary can help to return the lowered organs such as the bladder, uterus and/or rectum to their original position.Thanks to its suction cup effect, the cube pessary holds better than other pessary shapes, although the perforated version is easier to handle, but also sits less firmly.

Cube pessaries are also suitable for loosening scars and widening vaginal narrowings.They can be used for problems with bladder emptying, discomfort during sexual intercourse and before planned operations.Due to the flexibility of the silicone, the pessary can usually be inserted and removed easily by yourself.

Use

During the initial examination, the treating doctor adjusts the pessary.After initial instructions from a doctor or other specialist, the wearer can remove the pessary herself every evening and reinsert it in the morning to give the vaginal walls a rest overnight.In some cases it may make sense to wear the pessary at night, but only if ordered by the treating doctor.In the event of a major prolapse, a smaller pessary can be inserted at night for patients requiring care, always in consultation with the doctor.

To insert the pessary, an ointment can be applied over 2-3 cube edges to achieve a gliding effect and, when using estrogen, to promote better tissue blood flow or the establishment of normal vaginal flora.The choice of cream or ointment should be discussed with the attending gynecologist.

To insert the cube, you place your foot on a chair or the edge of the bed, similar to inserting a tampon. It may also be sufficient to insert the pessary with your legs spread, if necessary leaning against a wall or lying down.You push the cube as far into the vagina as possible.Only then will the pessary sit correctly and not cause any pressure problems.To remove it, after releasing the vacuum (back and forth movement), pull the cube down on the thread - ideally in different directions, if necessary with light pressure - until resistance is felt by the pelvic floor muscles.The accessible edges of the pessary must be moved with the index and/or middle finger so that the existing vacuum is released.The pessary should then be removed while maintaining gentle suture tension.If the patient cannot urinate, but also if incontinence is increased by the pessary, the pessary should be removed and a smaller (different) model chosen.The patient should be instructed to immediately report any symptoms - including urination/defecation - during pessary therapy.
