Urinary stone diseases

Up to 5 percent of the population in Germany is affected by urinary stone disease.The causes vary depending on the type of stone.Due to metabolic disorders, high concentrations of poorly soluble salts can occur in the urine and thus crystallization.The pH value of the urine also plays a role.A urinary stone can be as small as a grain of grain, but can also be the size of a chicken egg and larger.

Urinary stones can cause symptoms ranging from mild, barely noticeable pain to very severe pain (colic).Not every urinary stone triggers colic.Smaller deposits are not initially noticed.Although most stones are not very large, they cause significant pain because of their sharp-edged crystal structure.If a stone from the kidney gets into the ureter, it can cause sudden and repeated cramps (colic).Depending on where the stone is located, the pain radiates to the back, groin, thighs and genitals.Nausea and vomiting during colic can be distressing companions.In any case, an examination by a doctor is necessary.

Urinary stones often pass spontaneously when urinating.Knowledge of the composition of urinary stones is of great importance in determining the causes of stone formation.Every urinary stone should be analyzed carefully.The urine can be filtered to find lost stones.Have the urinary stones analyzed and follow your urologist’s treatment recommendations!

Urinary stone disease is a chronic disease of the entire organism.Depending on the stone composition, patients often form urinary stones again without preventive measures (high recurrence rates of 30-70%).

General measures to prevent new stone formation

Through sufficient fluid intake (2.5 to 3 l per day), through a healthy, varied diet with reduced amounts of animal protein (stone-specific nutritional recommendations can be requested from Blemaren Service Aristo Pharma), regular sporting activity such as cycling, gardening, climbing stairs and walking, weight normalization.

Uric acid stones (frequency approx. 10-15%)

Only uric acid stones can be dissolved and new formation prevented by medicinal alkalinization of the urine (e.g. with Blemaren N).

Calcium stones (frequency approx. 70-80%)

Calcium stones cannot be dissolved with medication.Alkaline citrates (e.g. Blemaren N) can prevent new formation and/or the increase in size of remaining fragments in calcium stones.

Area of application

Blemaren N effervescent tablets are approved for the alkalinization of urine in cases of uric acid stones with or without concurrent calcium stones and for metaphylaxis (follow-up care) for calcium stones (prevention of new formation and/or prevention of the increase in size of remaining fragments).No prescription is required for this product.

Active ingredients / ingredients / ingredients

1 Effervescent tablet of Blemaren N effervescent tablets contains: active ingredient: citric acid 1197.0 mg, sodium citrate 835.5 mg, potassium hydrogen carbonate 967.5 mg.Other ingredients: lactose monohydrate, mannitol (Ph. Eur.), lemon aroma, saccharin sodium 2 H2O, adipic acid, macrogol 600.

Contraindications

The medicine must not be taken if you are hypersensitive to citric acid, sodium citrate, potassium bicarbonate or any of the other ingredients of the effervescent tablets, if you have impaired kidney function, if you suffer from urinary tract infections with urea-splitting bacteria (risk of struvite stone formation), if you suffer from metabolic alkalosis, if you have a rare hereditary disease with increased potassium levels (Adynamia episodicahereditaria).Children under 12 years of age should be excluded from treatment as there is insufficient experience for this age group.

Dosage

Always take the medicine exactly as directed.Please ask your doctor or pharmacist if you are not sure.Unless otherwise prescribed by the doctor, the usual dose is: The following applies to the alkalinization of the urine in cases of uric acid stones: The pH value of the fresh urine must be adjusted to the range of 6.2-6.8 to prevent recurrence (recurrence prevention or metaphylaxis). The daily dose required for this can be different for each patient.As a rule, a daily dose of 3 effervescent tablets, divided into three equal doses throughout the day, is indicated.If the daily urine pH profile falls below pH 6.2, the dosage should be increased accordingly.Such a correction should preferably be made by increasing the evening dose to 2 effervescent tablets, in exceptional cases to 3 effervescent tablets.If the pH value of 6.8 is exceeded, the dose must be reduced.Such a correction should also preferably be carried out with the evening dose.For the chemical dissolution of uric acid stones (chemolitholysis), the pH value of the fresh urine must be adjusted to a range of 7.0-7.2.

The following applies for the metaphylaxis of calcium stones and for the alkalinization of urine in uric acid stone sufferers with concurrent calcium stones: A pH range of 6.8-7.4 must be set for the fresh urine.To reach this pH range, an individual dosage is required, usually 3 effervescent tablets.The dosage can be increased if necessary, although 5 effervescent tablets are usually sufficient.A daily dose of 3 effervescent tablets can be taken as a single dose in the evening or in three equal doses throughout the day.If a higher dose is required, it is recommended to divide it into three individual doses, each with one effervescent tablet in the morning and at lunchtime and 2-3 effervescent tablets in the evening.

According to the doctor's instructions, the medication must be dosed according to its effect, i.e. the individual dosage must be determined that brings the urine into the optimal pH range.The effectiveness is usually checked three times a day by measuring the pH value in the urine.Please talk to your doctor or pharmacist if you have the impression that the effect of the medicine is too strong or too weak.

Depending on the size of the stone, treatment times of four weeks to six months are necessary to dissolve the stone.The duration of use must always be determined by the treating doctor.Individually determined interval therapy has proven to be effective in preventing recurrences.

An undesirable influence on metabolic-physiological parameters is not to be expected even at doses higher than the recommended one if the kidney function is sufficient, since the excretion of an excess of base by the kidney represents a natural regulatory mechanism for maintaining the acid-base balance.A possible overdose can be detected at any time by checking the urine pH range and corrected by reducing the dose.If necessary, medical advice should be sought.

Do not take a double dose if you forget to take the previous dose.Please continue taking it at the scheduled time according to your doctor's instructions.

Please adhere to the duration of therapy specified by the doctor, otherwise complete treatment success cannot be guaranteed.

Intake

The effervescent tablet is dissolved in a glass of water and drunk immediately after dissolving.Please stir gently.A slight clouding is possible and small insoluble components can settle on the surface.

Patient information

Particular caution is required when taking it: Before the first treatment with the preparation, adequate kidney function must be ensured and elevated electrolyte levels in the serum must be ruled out.If renal tubular acidosis (RTA) is suspected, the acid-base status should also be checked.If you suffer from conditions that can promote the occurrence of urinary stones (e.g. adenomas of the parathyroid gland, malignancies associated with uric acid stones), these should primarily be treated with causal therapy.Regular checks of urine and blood values should be carried out during treatment.Particular attention should be paid to the acid-base status.To support the effect of the medicine, you should reduce your salt intake during treatment.A daily drinking amount of approx. 2-3 liters in the form of tea, fruit juices or alkalizing mineral water is necessary in order to reduce the risk of stones by producing a sufficient amount of urine.If you suffer from severe liver dysfunction, you should use the medicine with caution.

The medicine has no influence on the ability to react.

If there is a simultaneous uric acid metabolism disorder, allopurinol preparations can be given together with effervescent tablets.During use, foods rich in purines (e.g. offal, sardines) should be avoided.A daily drinking amount of around 2 liters in the form of tea, fruit juice or alkalizing mineral water is necessary.Before using the drug, the mineral levels in the serum should be determined and the kidney function should be checked.An increase in the potassium concentration outside the cells reduces the effect of cardiac glycosides; a decrease increases the arrhythmic effect of cardiac glycosides.Aldosterone antagonists, potassium-sparing drugs for dehydration, ACE-inhibitors and painkillers without cortisone reduce potassium excretion via the kidneys.It should be noted that one effervescent tablet contains 380 mg of potassium ions or 9.7 mmol of potassium.If you are on a low-sodium diet, it should be noted that 1 effervescent tablet contains 220 mg sodium ions or 9.7 mmol sodium.
