Area of application

This preparation contains the active ingredient sodium hydrogen carbonate and is available in the form of gastro-resistant soft capsules to be taken orally.It belongs to the group of antacids and is used to treat metabolic acidification of the blood, also called metabolic acidosis.It is also used as maintenance therapy to prevent the recurrence of metabolic acidosis in chronic kidney failure, i.e. impaired kidney function.

Each soft capsule contains 500 mg sodium bicarbonate.The following ingredients are also included: 136.83 mg sodium ion, 363.17 mg hydrogen carbonate ion, 3-sn-phosphatidylcholine (from soybeans), iron(II,III) oxide, Eudragit L 100-55, gelatin, glycerol 85%, glycerol monostearate, hypromellose, macrogol, sodium dodecyl sulfate, polysorbate 80, 8 mgPropylene glycol, refined rapeseed oil, hydrochloric acid 25%, partially hydrogenated soybean oil, 15 mg hydrogenated soybean oil, 50 mg partially dehydrated sorbitol solution, talc, titanium dioxide, yellow wax and purified water.In total, each capsule contains 137 mg sodium ion, which corresponds to 6.5 mmol.

Contraindications

The medicine must not be taken if you are allergic to sodium bicarbonate or any of the other ingredients.It is also contraindicated if the alkali level in the blood is too high (metabolic alkalosis), the potassium level is too low (hypokalemia) or the sodium level in the blood is too high (hypernatremia).In addition, the medication must not be taken by children under 14 years of age.

Dosage

Always take this medicine exactly as described or as directed by your doctor or pharmacist.The usual recommended dose is between 3 and 5g of sodium bicarbonate per day, which is equivalent to 6 to 10 soft capsules.The exact dosage depends on your blood values and is adjusted according to the severity of the acidosis.If the pH value in the blood is very low (below 7.2), an infusion is more suitable.

There are no data on use in adolescents.Do not take the medicine for a long time without medical supervision, as it may cause high levels of sodium or low acidity in the blood.

In the event of an overdose, symptoms such as dizziness, muscle weakness, tiredness, bluish discoloration of the skin or mucous membranes, shallow breathing and cramp-like muscle twitching may occur.In severe cases, lethargy, confusion, intestinal obstruction and circulatory collapse may follow.The treatment consists of balancing the fluid and electrolyte balance.If you suspect an overdose, contact your doctor.

If you forget to take a dose, do not take a double dose.Tell your doctor about this.Do not stop taking it without consulting your doctor.

Intake

The soft capsules should be taken whole with a little liquid.Do not chew or open the capsules as this destroys the gastric juice resistance.

Patient information

Talk to your doctor or pharmacist before taking the medicine.The effect of the drug is initially monitored every one to two weeks, especially at higher doses.The electrolyte levels in the blood plasma are also checked regularly.Particular caution is required if you have shallow breathing, low calcium levels or high salt levels in the blood.

Each soft capsule contains 137 mg of sodium, which is 7% of the recommended maximum daily sodium intake for adults.The maximum daily dose of 10 soft capsules contains 1.37 g of sodium, which is 70% of the recommended maximum daily sodium intake.Talk to your doctor or pharmacist if you need three or more soft capsules over a long period of time, especially if you are on a low-salt diet.

The safety and effectiveness of the drug in children and adolescents have not been studied.Therefore, it must not be taken by children under 14 years of age.

The medicine has no influence on the ability to drive or use machines.

Pregnancy

If you are pregnant or breast-feeding, think you may be pregnant or are planning to have a baby, ask your doctor or pharmacist for advice before taking this medicine.There are no studies on taking this medicine during pregnancy and breastfeeding. Although there are no objections to taking sodium bicarbonate if indicated, it should only be taken during pregnancy and breastfeeding after consulting your doctor.

A blood pH below 7.2 requires correction of the acidification of the blood through infusions.The effect of the drug must be monitored initially and especially at higher quantities at least one to two weeks apart, e.g. by measuring pH, standard bicarbonate and alkali reserve.The levels of minerals in the blood, especially sodium, potassium and calcium, must also be checked regularly.Depending on the results of these checks, the additional quantity must be determined.If long-term use is well adjusted, occasional checks are sufficient.
